选成C了，题目有描述：equally well-funded and comprehensive systems 并且C如果作为论点的话，只能说明两个国家local sructure所提供的帮助不同而已，切记强行脑补

其他都跳出范畴 C理解成护理有不同，因为当地基础建设 但其实就算这样 不能自己脑补原因 也可以是不同导致S的更少啊

E选项：题干明确提到both informal assistance and formal assistance, 可见assistance总共分为两类，也就是此消彼长的关系；同时提到formal assistance的资金和系统两个国家都是一样的，也就是说formal assistance提供能力是一样的，但是为何一个更多，那就是informal assistance缺少导致必须要主动补偿。仔细去理解就是这样，但其实本题做法主要是排除无关选项。
