通过its就可以判断出来，前面是一个单数名词，BDE排除
且在曼哈顿语法中，rank as 是正确用法，have the rank of 是错误语法

糖尿病，单数，要用三单，BDE错。剩下AC，动词ranks as大于名词the rank of。
看到副词only位置，题意表示糖尿病的高死亡率，修饰的应该是它的不可超越性，表示仅仅被heart disease and cancer 超过，only要在这两个疾病前。
only surpassed只有超越，修饰动词去了，与题意不符。

而这里的改变意思很好解释，SURPASSED ONLY BY是说它仅被癌症和心脏病超越，说这个病威胁很大，超过它的只有两种病。
而ONLY SURPASSED BY是说，只是，它还是被癌症和心脏病超过了，这样说可能不太好理解，换个例子，只是，这种疾病的死亡率还是被感冒和发烧超过了，说明这个病其实不是很厉害的一种病。

非常同意你的关于Only位置的第一个解释，即surpassed only by 强调只被癌症和心脏病超越，其它的病都比不过它。而第二个，我在你的解释上有所补充，我认为可以这么解释。only surpassed by 用被动可能难以理解，换成主动就更容易理解了。heart disease and cancer only surpassed D这个病，强调的是心脏病和癌症只超过了它，别的什么病都没超过。让我感觉这些病都好菜呀。

1）就近原则：either...or...；neither...nor...；not only...but also...；not...but...；there be句型
2）就远原则：(together/along) with...；as well as；but/except/besides；rather than；like等
